Meeting Minutes
March 31, 2008
Memorial Town Hall

MEMBERS PRESENT:  Martha Waite, Mark Johnson

OTHERS PRESENT: Andrea Crete, Agent, Randy Mizereck, Consultant, Debra Sciascia, Recording Secretary

The Board of Health Meeting was called to order at 5:04 PM.

SEPTIC PLAN REVIEWS:

305 BRYANT ROAD: Thompson Liston Associates Inc. for Majcher Family Trust. Complete system repair. The applicant is aksing for a 3' separation from the SAS, and is using a 40 mil liner.
Conditions:
1. Need local upgrade approval form.
2. Locate the present septic system on the plans.
Tabled for more information regarding calculations on the 3' separation from the SAS.

CHAPIN ROAD, Lot 13: L.A.M. Builders is requesting a one (1) year extension on the approval for the septic plans.
The Board agreed to grant the extension with the stipulation that the conditions and recommendations from the approval letter dated March 29, 2005 remain in effect and that the septic design plans must comply with revised Title 5 regulations including but not limited to:
• Two (2) additional deep holes
• magnetic tape needed on all system components
• inspection ports are required
• risers over d-box and septic tank if finished grade greater than 9"
Conditions:
1. If any part of the retaining wall is more than 4' above grade, it is required that the wall be designed with engineered drawings.
M. Waite motioned to grant a one (1) year extension only, with no further extensions. M. Johnson seconded. All in favor.

MEETING MINUTES:
• February 25, 2008: M. Waite motioned to approve the minutes. M. Johnson seconded. All in favor, approved.

OTHER BUSINESS:
• Abandoned Properties: A. Crete will check with the light department and the assessors office to see if she can be notified when a property is abandoned.
• Smoking at Wachusett Regional High School: The 'no smoking' signs were posted at the high school. Board discussion.
• Disposal Workers Installation Permit: An applicant has applied for a installation permit to install a Presby Pump System, one of the most difficult systems to install, without any previous experience. He has taken the Presby class, and A. Crete has the Nashoba Board Licensing test that he could take. It was the general consensus of the Board to deny the license due to the fact that the Town of Holden requires copy's of 3 licenses from other towns, or 3 letters of recommendation for installed systems. The applicant only has 2 letters of recommendation, but not for the installation of septic systems. Denied.
